b. Hearing about natural disasters I worry they might happen here.
c. I worry a lot about natural disasters and constatn.
d. I'm terrified of certain natural events like storms, earthquakes or floods.
8. a. I'm usually comfortable in my surroundings.
b. I am more aware of my surroundings in some situations than in others.
c. I'm more aware of what's going on around me than others. It's safer.
d. I'm very aware of what's going on around me at all times. I'm constantly scanning the environment for possible threats.
9. a. I've been in bad situations. They don't bother me now.
b. Some situations remind me of a bad event and how I felt then.
c. Places where I was involved in a bad event make very anxious.
d. Many things remind me of bad events I was involved with. They make me so panicky I try to avoid them if possible.
. 10 a. I'm comfortable with my thoughts.
b. Sometimes odd thoughts pop into my head that make me anxious e.g. about germs, concern haven't unplugged the heater, committing acts of violence, or of something bad happening to my family.
c. I frequently have the thoughts described in b. To relieve the anxiety I have to perform a ritual like wash my hands and repeatedly check things.
d. I'm consumed with the thoughts described in b. and with rituals I have invented to make things ok. These take many hours a day,
11. a. I'm confident I can manage in most situations.
b. I sometimes get anxious when I'm outside alone or in a crowd.
c. I often get very anxious outside alone. Crowds make me anxious.
d. I so terrified of being out alone I can't leave the house.

c. I often use substances to help manage my anxiety.
d. I depend on alcohol, cigarettes, medication or drugs to manage my anxiety. Nothing else helps.

b. I sometimes feel keyed up and have trouble concentrating.
c. I'm often anxious and worried. I get tired easily. I'm restless and have trouble concentrating.
d. I'm so anxious, fatigued, irritable, and distracted, it's hard to settle to anything and complete tasks.
14. a. I'm not usually bothered by a sudden loud noise.
b. Sometimes a loud sudden noise startles me for a minute.
c. Sudden noises make me jump. I don't like them.
d. A sudden noise, loud makes me jump and get panicked.
